This Order form does not do any calculation or estimation. It only e-mails your information to us through a secure socket layer. You will have to do your own math using the conversion chart below.
The order form works for patterned and non-pattern pads. You can fill it out and e-mail it to us by clicking the submit button or you can open up the .pdf file and print a copy for mailing or faxing. Dining table pads up to 54" by 74" without the leaves are $149.00. Leaf pads up to 24" by 54" are an additional $35.00 each. Oversize charges are $3.00 per linear inch in one or both directions.

This does not apply for leaf storage bags ordered without pads.

For Dining table pads, $28.00 will get you a regular 4 to 6 week delivery. For a 2 to 4 week delivery, an extra $20.00 rush fee will be charged in addition to the shipping and handling fee. Check this box " " if you want to pay the rush fee.

The packing and shipping for buffet pads, or coffee table pads is $14.00.
